Is this a worrisome sign? Reply by Dr.Sreekanth Reddy, Nipple discharge is the third most common symptom patients presents to us at the OPD. There are various kinds of Nipple discharges. The physiological Nipple discharge is the milk. Other than milk secretion is pathological. Patients can complain of yellow color or straw colour fluid coming out form nipple or discharge from pus. Sometimes this yellow discharge cam be mixed with pus or there can be frank blood discharge also. There are various factors that we need to consider before evaluating this. We need to see that is the kind of discharge, the quantity, whether it is is presents on the single side or both sides of the breast and we need to see any associated features like if there is any mass present in the breast or any skin changes like any ulcer or any discoloration of the skin or any itching over the nipple area. So all these associated factors also need to be looked after . So based on these findings we will be evaluating the Nipple discharge. Most of the times, 80% of the times the Nipple discharge is non cancerous in nature. It can be just a small infection or any benign conditions of the breast. Only 5 to 15% of the times they can be related with cancer condition. So how do you evaluate the Nipple discharge? First the physician evaluates for presence of any lumps in the breast or the axilla . After that there will be investigations. Whatever discharge is coming out of the breast will be examined under the microscope to look for any oncological cells if present. There is a spherical tests called as ductoscopy where a small camera will be out through the nipple area through the ducts to look for any pathology. We can also ask for any investigations like ultrasound or mammogram to look for any lumps or any precancerous or cancerous conditions which could have been missed out clinically in a non palpable case. If you notice anything then don’t panic. Breast lump can be seen due to many other reasons. If the lump is in the armpit region it could be an enlarged lymph node due to any recent viral infections, and scratches or cuts on the hand or arm. Breast lump that feel squishy, like a soft rubber ball, are more likely to be benign. But, if the lump feels firm, fixed and can't be moved around under the skin then you should consult your doctor 2. Check If you have breast pain or pain around the nipple area for long time. It could be an early sign of breast cancer. 3. Check if there is any change of your nipple shape and size. Breast cancer can cause inverted or retracted nipple. 4. Check if there is any discharge from one breast that’s clear, red, brown, or yellow other than milk secretion. This could be due to many other reasons like infection, cyst, papilloma. However, this could be one of the warning signs of breast cancer. You can check the colour of the discharge with the chart mentioned in the video 5. If you find any unexplained redness, swelling, skin irritation, itchiness, or rash on the breast then contact your doctor. 6. Next important sign is changes in skin texture. It could be a thickening and swelling of a part of the breast or dimpling of breast skin. Don’t confuse it with age related creases. If it is related to age it can be seen evenly on both the breasts.
